Now that their residencies are over, J.D., Elliot and Turk are moving on to the next phases of their medical careers. Elliot is starting a cross-sectional imaging fellowship, where she'll be looking at bone biopsies. J.D. thinks he's going to be named chief resident by Dr. Cox. But that outcome could possibly change since Elliot throws her name in for consideration at the very last minute.
Cox makes a surprise decision by naming both Elliot and J.D. joint chief residents. To add to their already tense relationship, they have to share an office the size of a tiny closet.
Meanwhile, Molly Clock uses Dr. Cox as a guinea pig for research on a paper she's doing on conflict resolution between hospital personnel. She also uses conflicts between J.D./Elliot and Turk/Dr. Cox, who can't agree on how to remove a lightbulb stuck in a patient's lower g.i. tract, and her own issues with Carla as examples in her paper.
The Janitor provides Turk and Cox with some important information to help them remove the lightbulb; Carla and Molly resolve their issues about being the "go to" person when other people need advice
